Faith Formation Registration Form

Forms:

There are 2 registration forms for each family to complete, the general form below and a Confidential Release Form that requires a parent's signature. Both forms can be printed out with the button above and turned in, or the general form can be emailed below to the office and the Confidential Release form printed and turned in. Fee payment should be returned with the forms or as soon as possible after both forms are completed.


Fees*:

Families with kids 4 yrs old - 9th grade_______________________$25 X # of students (max cost: $75 per family)


Additional Sacramental Fees______________________________$25 X # of students

  (Grades 2, 3, 9)


*Need-based scholarships are available. Contact our office for information. No one will be refused admission for inability to pay.

Make checks payable to Beloved Disciple Parish. Return payment with forms as soon as possible to Beloved Disciple Parish in Grove City, to the attention of Lisa Bablak, Faith Formation Secretary.

Faith Formation Registration Form

Please indicate the number of children who will be entered in sacramental preparation sessions here (there is a separate $25 sacramental fee for each student). There will be exta sacramental preparation classes in addition to the Faith Formation classes, and one retreat is also required for sacramental students.
